Treating melasma in Malaysian skin: why aggressive lasers can backfire.

Melasma is one of the most common, and most misunderstood, pigmentation conditions we see in Penang. It appears as symmetrical brown or grey-brown patches, usually on the cheeks, forehead and upper lip, and it disproportionately affects Asian skin. Patients often arrive having already tried strong creams or aggressive laser sessions, frustrated that their pigmentation came back darker than before.

That rebound is not bad luck. It is biology.

Why melasma behaves differently

Unlike sun spots, melasma is a chronic, relapsing condition driven by overactive pigment cells that are exquisitely sensitive to three triggers: ultraviolet and visible light, heat, and hormones. Darker skin tones, Fitzpatrick types III to V, which describe most Malaysian skin, carry more reactive melanocytes to begin with. When an aggressive laser delivers too much energy, the very inflammation it creates can switch those cells into overdrive. The result is post-inflammatory hyperpigmentation layered on top of the original melasma: a darker patch than you started with.
