Botox or fillers for fine lines? How doctors actually choose.

The two most requested injectables solve different problems. Here is the decision framework our doctors use, so you understand your plan before you sit in the chair.

The short answer

Botox treats lines caused by movement; fillers treat lines caused by volume loss. Look in the mirror and raise your eyebrows or smile: if the line deepens with the expression, it is a dynamic line and botulinum toxin is usually the first tool. If the line is visible even when your face is completely at rest, it is a static line, and that often calls for filler, a bio-stimulator, or skin-quality treatment instead.

When Botox is the right tool

Botulinum toxin relaxes the small muscles whose repeated contraction creases the skin: forehead lines, frown lines between the brows, and crow's feet. Treated conservatively, expressions remain natural while the crease softens over 3 to 14 days, lasting around 3 to 4 months. It is also the tool for jawline slimming and a subtle lip flip. What it cannot do is fill a groove that exists at rest: relaxing a muscle does not replace lost volume.

When fillers are the right tool

Hyaluronic acid fillers restore structure and volume: nasolabial folds, marionette lines, cheeks that have deflated with age, lips, chin and jawline definition. A good result looks like rested anatomy, not added bulk. Fillers are measurable, precise, and importantly reversible: hyaluronidase can dissolve them within minutes if ever required.

The cases where the honest answer is neither

Very fine crepe-like lines across the cheeks or around the mouth often reflect skin quality rather than muscle or volume. For those, skin boosters (Profhilo, Rejuran), collagen bio-stimulators, or laser resurfacing frequently outperform both Botox and filler. This is why a proper assessment matters more than choosing a product from a menu.

1. Motion test. Does the line appear with expression or exist at rest? 2. Pinch and recoil. Skin quality and elasticity determine whether the skin itself needs treatment. 3. Volume mapping. Conservative sequencing routinely saves patients money compared with treating everything at once.

Costs, honestly
